As a specialist provider of complex care services, we understand the challenges that individuals face when living with a long-term conditions.

Often confronted with a lack of real choice, reduced independence and a feeling of isolation, the future can seem challenging.

Solution2Care Services delivers specialist complex care to both Adults and Children who are suffering with severe disabilities and complex health needs in their own homes. Our homecare service allows those suffering with life limiting and long terms conditions to stay within their own home to receive ongoing treatment and support. We work in partnership with both the private and public sector to deliver high quality person centred care.

We provide home care to a range of individuals with a full spectrum of conditions and needs that include:

We are able to supply a range of Registered Nurses, Complex Care Worker and Support Workers with specific skills that are tailored to the types of complex conditions our service users have. We also have a strong focus on training ensuring all of our staff are trained not only to national standards, but when relevant, receive bespoke service user specific training.

The main features of our services are:

Nurse lead packages of care

All of the packages that have clinical needs are overseen by a Nurse Specialist. They ensure that the package of care is regularly reviewed and managed effectively.

Recruitment of a highly skilled workforce

We combine the utilisation of experienced staff already on our existing workforce as well as recruiting new bespoke teams. Using either method we ensure that we attract workers with relevant skills and experience as well as matching the service user’s preferences.

A full implementation plan prior to commencing care

To ensure that our packages run smoothly we always put together a full implementation plan which is shared with the wider multi-disciplinary team. The plan includes details around the initial assessment, recruitment and training.

Delegation of clinical tasks

We provide a combination of bespoke theoretical training alongside practical competency based training to enable Health Care Assistants to carry out clinical tasks with confidence. This is continually monitored, reviewed and adapted to meet changing needs.

The Solution2Care Services team have a diverse range of skills and proven experience in the delivery of complex Home Healthcare packages. Our service user’s typically have a primary health need and have care requirements that are complex or intense and/or unpredictable.
